Founded in 2009, the Greater Buffalo United Accountable Healthcare Network (GBUAHN) is one of the NYSDOH designated Health Home in Erie County. GBUAHN’s Health Home provides Medicaid recipients care coordination services through an integrated delivery model which incorporates medical, behavioral, addiction and social service in a single stream of delivery.
GBUAHN’s Health Home services provide patients with an extra support system to ensure they have the resources they need to stay healthy. Care Teams meet with members in their communities, establish relationships with them, and assess the “bigger picture” of the patient’s health needs. The Care Team members connect personally with the patients to help “connect the dots” and link them with the services, resources, and providers they need.
